Terms and Conditions: I acknowledge that this athlete has been examined by a physician within one (1) year to compete in athletic activities. I hereby give my consent for the above athlete to participate in the Track Phi Track Elite Program. I, the parent/guardian hereby waive and release any and all claims I may have against Track Phi Track Elite, Inc., its Officers, coaches, and agents or it's representatives from any and all liability due to personal injury resulting from activities sponsored by Track Phi Track Elite, Inc. or for which Track Phi Track Elite, is a participant. I authorize the coaching staff of Track Phi Track Elite to make any decisions concerning the health, welfare and safety including medical treatment for this athlete during my absence. I have read and understand this information and information included in the registration packet.
